The primary health care strategy focuses on bringing basic health services to all people by using community participation to reach people more effectively and to stimulate a sense of individual and community responsibility for preserving and improving health. Yet countries have encountered serious problems in trying to implement the often vague and misunderstood concept of community participation. This publication is an analysis of selected case studies from Barbados, Brazil, Colombia, Cuba, Ecuador, Guyana, Jamaica, and Mexico that were conducted to determine why, when, and in what ways community members voluntarily participate in promoting health and what sustains this participation over time. The lessons learned from this comparative analysis provide a basis on which countries can expand their efforts to deliver health care at the community level and promote greater community responsibility in health care decisions.
